The Winter worries me" - By this do you mean the Winter worries both you and the person you're speaking too in this piece? Or, do you mean the Winter worries you like the other person worries you?

And in the grave" - What is cold? the hand you long to hold by the fire? Forgive me if I'm being stupid!

For your second question as long as you following the punctuation you'll see that's one long sentence; so yes, the hand is cold AND in the grave (because the person is dead). but the narrator still wishes to hold it by the fire.
Hope that cleared up confusion. Do you these parts are too obscure for the reader?
