The pain we go through is often unbareable but often not understandable either so we question who knew or who knows when even we did not know

"
Who knew it could hurt so much?
That first smile.
That first kiss.
That first thing seeming worth while.

Who knew it could hurt so much?
That first sign.
That first love.
The first time you lied saying you were fine.

Who knew it could hurt so much?
The first tears.
The first fight.
The first time facing your fears.

Who knew it could hurt so much?
The first scar.
The first drop of blood.
The first time not knowing who or where you are.

Who knew it could hurt so much?
That first goodbye.
That first time you knew.
The last of all the lies.

That only you could realize it hurt.  
That last touch.
That first realization.
But who knew it could hurt so much?

Great, great, great write! Very emotional, but clear.

The one thing I would say is you might want to re-word this lines, "That only you could realize it hurt." It is just a little awkward. I had to stop in the middle which was distracting.

OTHERWISE! I love it. Truly.

Great repetition, and the rhyme scheme was very cool. The whole thing seemed to be telling a story. A skeleton for our imaginations to put flesh to.

I really do love this.. How can something so beautiful become so hurtful? We never find out until it's too late.

This write was very clear, very touching, and certainly very emotional. You demonstrated the hurt and betrayal so well. Wonderful job:)

The repetition works seamlessly with the poem's theme. The only line that I would suggest tweaking would be "The first time not knowing who or where you are" simply because it's a bit of a mouthful.
