This is where thoughts go:

D

O

W

N

And 

S   p   i    r    a   l   i    n       g   …

               To the
Left.

Because when did my mind ever revolve around anything that was                                                   right?                           

Day and night,

They haunt me,

Want me unwanted,

D   e   b   r  i    s  in the sea,

Lost and losing.

These volatile voices-

They are my own;

But not of my choosing.

Many penny voices telling me:

You are worthless
You have heard this
All before
In the context of evermore
As we come back
To the front
Of your mind confronted;
Words you wish never heard
Echoing from the dumpster of dreams deferred.

You have perfectly and effectively described psychosis. Schizophrenia can be ravishing and horrific if not treated. Very beautifully, and skillfully done!
