"Hurting, I'm hurting" You're hurting from something - physical pain? Emotional pain? Spiritual pain? Mental pain?
"I want to scream so bad" The pain is so intense you want to cry out for help, just so someone will hear you, though you're not sure if you want to for sure.
"Please, help me" You're asking for help, pleading for someone to find your broken form and save you from your misery.
"I can't stop" Makes me think you're the one causing your own pain.
