I think by comparing the emotional pain to the physical pain of knives in the skin you created a whole new level of skill in the poem. I also like the way you set the emotional pain on a higher level than the physical pain. It makes it more relatable to the reader. Nowadays, people are not being physically harmed, but the emotional scarring that they deal with is worse than any physical pain. Nicely done.
