PSYCHIATRY TODAY: The "Counseling" Form...

1) How are you today?
  a) ask about mental conditions, especially progress or regression on previous diagnosis
     a) happy/manic?
     b) sad/depression
     c) anxiety/anxiety sleep problems?
     d) recent sleep about 8 hours a night and about every night?  
     e) stressed
     f) (and so on like up and down too much or just too up?)
  b)  How are you doing for money, lately, according to your needs and some for fun?  (no answer as too personal is fine, too!)  A
    a)  Also, you can help them apply for a better job.  Go to www.monster.com and post a resume.
  c) How is your health?  You can recommend doctors for a visit or, if you are a doctor/psychiatrist render aid!
  d) What abnormal conditions have you seen?  Psychiatrically or socially?  
     a) Losing a lot of friends?
     b) Recent death in family or circle of friends?  (condolences)
   e) How are your children doing?  (if doing poorly in school, suggest a "tutor" or teaching them what they need to know.)
  f)  Have you been fighting with your adult content partner/true love we hope?
  g)  Where are you living?  If the patient is living with their parents, this raises a "red flag."
  h) What other concerns do you have today?
You can use this form when calling a "counselor" or a "psychologist" or a "psychiatrist," and you can note on this form if the conditions change as a matter for your next visit.

Does this help?  Don't we do more than this form in "mental health," but how great is this?  What about turning them onto government services like money from Social Security for disability or government health care that is better than nothing?  Just some love?
Not to belittle your practice, but I can design more forms like this and put them on the Internet to send you an email of the visit!

EDITOR'S LATER NOTE:

This form can be filled out IN PRIVATE by a:
  1) Data entry specialist,
  2) Counselor before the psychiatric expert or by themself, or
  3) Psychiatrist or Psychologist.
NOTE: This form is presumed to lead to "bad bedside manners," but you can ad lib parts and pass out the form in the waiting room for something to do.  
Don't forget to ask for a change in contact information or health insurance in your waiting room form!
I hate to say it, but most of psychiatry or counseling or psychology can be handled this way, manual labor, form filling.
