tightening without realizing
struggling, breathing failing
a sick slow death
a poison

I think it is sad that you feel the need to explain what the poem is about in your notes at the top -- it would be a much more satisfying experience if you could trust the poem and the reader to come to that realisation together.

One image that comes to mind is the 'hedonic treadmil'': http://en.wikipedia.org/wiki/Hedonic_treadmill
This is the concept that the acquisition of luxury does not bring happiness... it just creates a space for another 'need' (more like a want) to possess you. Therefore you are always running after more hedonistic pleasure on the treadmill and, unlike real running, you don't even get fit out of it.

Imagery like this will help to direct the reader to the point of the poem without them being told. Well, it will help direct some readers... you can never write something that everybody will understand in the same way.
