About where the truth lies,
It's written in the scars,
That are running down my sides.
If you chance upon them,
Don't call me a freak,
My faith is already broken,
My heart already weak.

The rhyme scheme looks ok.
The feelings described here might be a picture of a particular frustrating moment.
I hope this is not permanent.

If this poem is literal and you bare the scars of some catastrophe, change your prospective and wear them as a battle wounded warrior no matter how they occurred.

However, I just noticed in your bio that some of your work discusses self-harm. If your scars are self-inflected I take back everything I said and beg you to come to your senses. I do not condone self pity and mutilation.
