I took this poem to mean a gravestone. That someone strong had passed away and that the headstone was now planted in the ground. I guess that's part of the beauty of poetry-people bring themselves and their experiences when they read so everyone gets something different out of it. A very strong and vivid write.

perennial stone heart....has been pretty steely at times...

no bending for love...it hurts down to the roots....
and the soil ruined....hard to let the love grow...

but new seeds help.
