I was careless.

I wanted to alleviate my thoughts,
My thoughts had turned against me
Conjuring up my past to haunt me
Just for the pleasure of plaguing me.

I was too occupied in my head,
and a half-hearted view around me,
That I didn't see it coming.

I stumbled and tripped over a rock, That said
'Forget-Me-Not'

I didn't know where I was going,
I was careless.

This is great!
The plot your poem has doesn't seem to have a resolution, which I love.
I think the idea of being careless and tripping over a rock that tells you not to do what has already been done is amazing.
I really love this.
