Your poem clearly communicates your care for someone who is suffering from a terminal illness. Even old age, in my opinion, is unnatural and a disease. And your goal of encouraging someone with positive words is very commendable. Whatever pain and stress we experience is made more endurable by a positive attitude. Your poem is well-written, lucid and emotional. Very nice.
